Summer Clerkship

INSTRUCTIONS FOR BELOW PDF PAPERS:

Please DO NOT forget to PRINT OUT these pages and bring them with you to the hospital or clinic where you are doing your clerkship. Every student must print out these pages and do this!

Page 5 of this pdf have to be filled out by the head person who you are doing your clerkship in your home country with grades of these skills.

Page 6 needs a stamp from the hospital and signature and stamp from your supervisor whom you are doing your clerkship with also grades need to placed of the skills.

Page 7 needs a stamp from the hospital and signature and stamp from your supervisor whom you are doing your clerkship, plus at the bottom you need to do an evaluation of yourself in your clerkship give yourself a grade in two spots and sign with your own signature the paper.

Page 8 you need to write a 400-500 word essay discussing about the clerkship was this experience professional or not in Nursing Care.

Page 9 you need to fill out your name and surname, write in the dates of the clerkship and this page needs a stamp from the hospital and signature and stamp from your supervisor whom you are doing your clerkship with.

 FINAL EVALUATION the grade is placed by the doctor who you are doing your practice with but where it says signature and stamp of practice coordinator this is for Mr Rakesh Jalali, MD, PhD  to sign. Please leave this blank.

 All pages need to be fill out as listed above. ALL 8 pages must be brought to Mr Rakesh Jalali, MD, PhD please also bring your record practice book to be signed off also by Mr Rakesh Jalali, MD, PhD. Please follow all these instructions so that you get a grade into USOS system.

Those students, who pass everything in June and don’t have re-takes  in September,  and will NOT be here in September please pass your books onto someone so they can pass it along to get it signed by Dr. Jalali and then it must be handed into the Dean’s Office before  September 15th Everything MUST be signed off. If this does not take place you will FAIL the year the Summer Clerkship is one of the requirements to pass the year.
